When Leadership Dysfunction Destroys Everything

Written by Evan Duke | Jul 29, 2026, 12:00:00 PM

I wish every leader could see what I witnessed last quarter – a company that let dysfunctional leadership spiral into an operational nightmare that cost them their best customers and key employees. Chaos reigned supreme. No one knew what was going on. Everyone was blaming everyone else for dropping the ball. When bad publicity hit, it was revealed spectacularly that no one really was innocent, but everyone actually played a big role in collectively dropping the ball.

As a fractional COO, I've seen this play out on more than one occasion. While there are definitely many examples in the news, all of these situations boil down to one root problem: an absence of competent leadership that has resulted in systemic issues that now threaten to jeopardize the organization's existence.

The reality is that organizational dysfunction in leadership stems from someone preventing the leadership from functioning properly. Whether it's constantly circumventing the CEO through devious means, the CEO not wanting to engage and handle the harder, less glamorous aspects of running an organization, someone who is sowing discord and backstabbing people above them, or a leadership team who simply reacts to everything as opposed to planning.

Key Operational Insights: The Anatomy of Leadership Failure

1. The Many Paths to Dysfunction

It's not as straightforward as we'd like because it's possible that a strong and otherwise capable CEO finds themselves mired in a dysfunctional organization. On the flip side, it's also likely that an incompetent and very poor choice for CEO lands in that position as well. An organization that possesses dysfunctional leadership can get there through any variety of paths, making it very hard to eliminate the possibility entirely.

The Warning Signs:

  • Constant circumventing of established leadership hierarchy
  • Leaders avoiding difficult conversations and decisions
  • Backstabbing and discord among leadership team
  • Reactive decision-making instead of strategic planning

2. The Predator Problem

Bad actors are predators focused simply on increasing their power without regard for anyone else around them. From sociopaths and narcissists to manipulators, there's a wide range of aberrant behaviors that belong to toxic individuals. These people will make the difference between functional and dysfunctional leadership.

3. The Alignment Crisis

Without clear focus on organizational goals and proper leadership alignment, dysfunction becomes inevitable. If you don't know where the organization needs to go, aren't actively planning to ensure it gets there, and lack a leadership team properly aligned with this vision, dysfunctional leadership becomes your default state.

Fractional Leadership Connection: External Intervention

The key takeaway is that leadership needs to be proactive, dealing with problems effectively, planning carefully, and focused on remaining aligned. Anything less will result in organizational dysfunction that could kill the organization.

  • Objective Assessment: Identifying dysfunction patterns without internal political influence
  • Bad Actor Intervention: Systematic approaches to handling toxic individuals
  • Leadership Alignment Systems: Ensuring unified direction and decision-making processes
